Please fill in this form. I will use your info to annotate the photos you sent by e-mail attachment.

Note: If you are simply changing a few items, fill in the Event Name Box and just those boxes that changed and "Submit".

Caution: Don't use the ENTER key. Move from Box-To-Box using your MOUSE. Move the Mouse Pointer to the Box you want and then Left Click, got it? If you do accidentally hit ENTER just use the "Back" button and then continue where you left off., no problem, but the page will be sent in, and I will know how many times you messed up.

We will put this up on the Web on the Review Page. If you find errors or things you want to change, just re-do the items on this form that need attention. Be sure to fill in the Event Name.
